Dr. Dominik Dubravec and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

Emergency/Single-Visit Payments


For single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders, credit card payment (MasterCard, Visa, American Express and Discover), and Care Credit.

Multiple Visit Payment Options

 

  • Option 1 : Full Payment at Time of Service – If paid in-full on the day of treatment, by check or cash only, a 5% courtesy is given for periodontal surgeries and/or scaling and root planing.
  • Option 2 : Pay Your Estimated Portion – If you have dental insurance, we will agree to let you pay the estimated responsible balance, and allow your insurance company to reimburse Dr. Dubravec for the remaining balance. This is only an estimate. If for any reason your insurance company does not pay the estimated amount, it becomes your obligation.
  • Option 4 : CareCredit – No initial payment to our office. Payment plans ranging from 3 to 60 months. Interest-free 6-month payment plan if financing $200-$1,000; or up to 12 months if financing more than $1,000. Pre-payments can be made anytime without penalty.
